A parent, lawful guardian, or authorized caregiver may submit an online support review request for a ward without first visiting a branch. We keep the request simple while protecting minors, privacy, consent, guardian authority, and clinical safety.

A parent, lawful guardian, or authorized caregiver can submit an online service request for their ward. The request asks for the parent/guardian full name, ward/child full name, ward/child age, relationship to the ward, contact details, service interest, and a child policy declaration.
For clients below 18 years, guardian consent is required and minor assent is recorded where appropriate. Minors cannot self-register through the public website.
